The culprit of the case is Shiori Hanzaki, whose real name is Natsumi Aogiri, Taketo Aogiri's younger sister. The murders however, were committed on impulse with no preparation in advance, and all of the tricks in the case were instead used by Sousuke Fuyube, the third victim, to cover up for her.
The motive of Shiori however, is not because of she herself wants to avenge her brother and parents, but because of "Natsumi Aoigiri" inside her wants to. After her brother Taketo's death due to the bullying, her mother Sanae suffered psychological trauma to the point that she tried to kill herself whenever she saw a sharp object. Though Natsumi and her father Takehiko tried to prevent her from that and to move on with their life after her brother's death, Natsumi had accidently left a fruit knife in the kitchen to gather the laundries, causing her mother to stab herself to death. Sanae was still and Natsumi picked up the knife and her body moved on its own and stabbed her.

Despair over his wife's death, Natsumi's father took her mother's body and her inside the car to commit suicide by drowning in the river, but Natsumi survived, lost her memories, and was found by the Hanzaki couples and adopted as their daughter with the new name of Shiori. Shiori, who lived a happy life with her adopted parents, completely forgot about who she was, until she unexpectedly ran into Fuyube and the rest again at the inn. Though she vaguely remembered about them being responsible for her brother's death, it was not until Onoda harassing her that Natsumi's memories and hatred returned, motivating her to kill them in vengeance for that "Natsumi Aogiri had already killed her own mother, so it doesn't matter if she kills more".
After Natsumi is arrested, Kindaichi visits her to reveal the reason why Fuyube tried to protect her from being the criminal even in his last breath: it was because he was truly sorry for causing the death of her family. Shortly after Taketo's death, Fuyube's parents committed suicide out of grief and he himself had a hard time. By chance, Fuyube stumbled into Shiori whom he immediately recognized to be Natsumi who he thought died along with her parents. Feeling responsible, he quietly financially supported her and her family from the shadow, but never revealed himself out of guilt. By the time that Shiori murders his friends, he was driven to despair and decided to let her end his life, but not before erasing every and any evidence that would point Shiori to being the murderer, and made it appear as he was the real mastermind behind the death of his friends as his last form of "atonement" to her.
Before that, Hajime tells Toramoto, when he was examining Fuyube's death, he realized Toramoto was a doctor and was one of the doctors who worked with the missing Ouryu Kigata and at the time Yoshino Aizen was a patient. Aizen explains that Kigata tried to kill her and Toramoto came to the rescue, when Kigata started to attack Toramoto, Aizen picked up his scalpel and stabbed in the back and they buried his body near the tree. Hajime decided not to tell since the Kigata case already passed the statute of limitations.
